Nominate Your Hero

On Saturday, June 29, 2013 District 5 United will celebrate our local heroes at a family event at Emma Prusch Farm Park. The event will include a family barbecue for the first 250 attendees and a Community Resource Fair. District 5 United is celebrating the accomplishments and good work that is happening all around our community. Once again we will be recognizing local heroes that are nominated by you. Community members are asked to submit an application describing your heroes. A hero is someone who has:

• Made a difference in someone’s life. This could be over a period (like a teacher, counselor, etc.) or in a specific event or situation (like a rescue).
• Made a contribution to the community via some specific action/activity (like organizing a Neighborhood Watch).
• Has repeatedly been a community activist making District 5 a better place to live and work
• A person who has made a lasting difference over time.

All nominated heroes will be notified and a handful of heroes will receive special recognition from District 5 United. 

Nomination Guidelines: A person will not be able to nominate themselves. Other than that, anyone can nominate anyone they choose. Nominations will be posted online in June. Please provide a photo. Photos will be posted with the nomination.
